Senior CommUnity Care is a new health care choice for older adults on Colorado’s Western Slope. Senior CommUnity Care is a provider of the Program of All-Inclusive Care for the Elderly (PACE) and is the first to operate in Western Colorado.

Older adults who want to live at home but need support
Caregivers caring for older adults living at home
Health professionals who want to help older adults living at home with a complete care system
